A rate sheet is a one-page PDF with your clinic info, services, pricing, and branding. DOT Lead Scout has a built-in rate sheet builder so you can create a professional-looking PDF in minutes — no design tools or printer needed. The finished sheet can be attached to outreach emails or shared as a public link.
Two Ways to Get a Rate Sheet #
Build one from scratch
Use the rate sheet builder — no design skills required. Pick colors, add services, preview, and generate a PDF.
Upload your own PDF
Already have a rate sheet you like? Upload an existing PDF (up to 10MB) and use it as your active rate sheet.
What You'll Configure in the Builder #
- Clinic info — name, address, phone, email, website, optional NPI number
- Logo — upload a PNG or JPG; placed in the header next to your clinic name
- Color scheme — primary (titles), secondary (table headers), accent (CTA strip)
- Services & pricing — one row per service with name, description, and price
- Feature boxes — up to four highlight boxes (e.g. "Same-Day Available", "FMCSA Portal Upload")
- CTA strip — the colored band at the bottom; title and subtitle are both customizable
- Additional info — free-text section for hours, policies, or anything else
Building Your Rate Sheet Step-by-Step #
Open the rate sheet tab
In the portal sidebar, click Rate Sheet. If you're on Engage+ you'll see both the builder and the upload zone.
Fill in clinic info
Most fields auto-populate from your profile. Add an NPI if you want it on the footer.
Pick your colors
Three color pickers control the primary, secondary, and accent colors. Match your existing clinic branding.
Add services
List each service you offer. Common ones: DOT Physical Exam, Drug & Alcohol Test, Audiogram, Vision Test.
Preview
Click Preview to see an HTML rendering of the final PDF. Adjust anything that doesn't look right and preview again.
Generate PDF
Click Save Rate Sheet. The PDF is generated, saved to your account, and set as your active rate sheet automatically.
Default Color Scheme #
Match your existing brand
If you already have a website, logo, or letterhead, copy those color values into the builder for a consistent look across all your patient-facing materials.
Your Active Rate Sheet #
- Generating a new rate sheet automatically marks the previous one inactive
- Uploading a PDF also replaces the active sheet
- Old rate sheets aren't deleted — they remain in your media library and can be reactivated if needed
- There's no charge for regenerating — you can iterate on the design as often as you want
Public link option
Every active rate sheet also gets a public URL (dotleadscout.com/rate-sheet/<your-id>) that you can share without going through email. Useful in text messages, social posts, or printed business-card QR codes.
Final PDF Layout #
- Header — logo, clinic name, "DOT Physical Examination Services" subtitle, contact info on the right
- Title bar — full-width primary-color band reading "DOT Physical Rates"
- Services table — each service with name, description, and price
- Feature boxes — if configured, a row of highlight cards under the table
- Additional info — if filled in, free-text section near the bottom
- CTA strip — accent-color band with your call-to-action title and subtitle
- Footer — NPI badge (optional), clinic name + phone + email, fine-print disclaimer
